Your records remain confidential. To request a copy for yourself, or to have them sent to a new provider, please email or call and include:
- Your full name and date of birth
- A phone number or mailing address where we can reach you
- Whether the records should go to you or to another provider (with their name and contact information)
Please don't email sensitive health details. A short request is enough, and we'll follow up to verify your identity before releasing any records.
